Pet Application for Residential Rental
I do hereby request that my pet(s) be approved to reside with me/us during the term of my lease agreement. If my pet is a “Service Animal” or a “medically necessary” animal, I will notify management in writing and supply appropriate paperwork verifying the validity of the accommodation request. In consideration of having a pet go through this application process, I agree to pay a non-refundable Pet Application fee of $35.00 to Main Street Property Management – due immediately and payable through the resident portal created when the initial prospect application was created.
NOTE: This is a Pet Application only and does not guarantee approval for a pet until approved by Landlord, a Pet Addendum is executed by all parties and all sums and fees/deposits are paid according to the Pet Addendum.
In order for the pet to be permitted on the premises, the pet must be approved by Landlord and Management, a Pet Addendum signed by Tenants and Management and all fees paid including but not limited to a higher security deposit, pet deposit, pet fee or additional rent as required by Landlord. I understand that Management is under no obligation to approve a pet for occupancy.
Pet fees and deposits are determined under the scoring system outlined below. This scoring is calculated based on: breed, age, weight, vet/vaccine records, pet references, spay/neuter records - with points awarded for the criteria. While aggressive breeds are accepted under the scoring system their is additional weight given to those breeds listed as "aggressive" under the Homeowner insurance policy.
- 5 Star - $20.00 per month pet fee; $0.00 pet deposit
- 4 Star - $25.00 per month pet fee; $0.00 pet deposit
- 3 Star - $30.00 per month pet fee; $150 pet deposit
- 2 Star - $35.00 per month pet fee; $350 pet deposit
- 1 Star - $40.00 per month pet fee; $500 pet deposit
